Wellness Assessments

Organizational Survey.
This survey is designed to solicit information from the senior management team that will aid in the development of a wellness program tailored to meet the client's needs. These data illustrate management perception of the current state of the organization.

Personal Wellness Survey
This survey is a health risk appraisal that addresses a wide range of personal information. The tool assesses the level and nature of the employee's health risk status. It provides the data required to provide specific recommendations to improve each employee's health and well-being.

Employee Interest Survey
This survey is given to the individual employees. It is submitted on an anonymous basis and allows for important data to be collected regarding the employee's interests in health care programs and services. In many instances, employers provide wellness programs that may or may not be of interest to their employee population. This survey provides data needed by the management team to ensure that they are providing the programs of interest to their employees.

Wellness Program Development and Evaluation

The results of the Biometric Screenings are used in combination with the Wellness Assessments to provide specific recommendations and to design a customized wellness program to improve employee health. Continued monitoring of the employee population is then done to evaluate the effectiveness of interventions and to adjust the program as needed. We also offer On-Site Wellness Coordinator Services for employers requiring assistance in implementing their wellness programs.


Nurse Wellness Coaching

Our Nurse Wellness Coaches are available for on-site consultation with human resource representative(s) and/or individual employees. They provide Individual Case Management Services for employees identified to be at risk, including but not limited to, Nutrition, Fitness and Stress Management, Tobacco Cessation, Chronic Disease Management and Family Support Services.


Kiosk Monitoring Stations

Technology that provides real time data is placed on-site for 24/7 access for the organization's employees. This technology measures weight, blood pressure, heart rate and oxygen saturation. The use of this technology allows employees to become increasingly knowledgeable about their own health status and begin to take increased accountability for their health.

Lifestyle Behavioral Programs

A lunch-n-lecture series can be provided monthly to address the health care concerns identified through the employee interest survey. These lectures are given by a nurse or another identified expert in the field.

Health Fairs, Workshops/Seminars and Newsletters

Health Fairs are an efficient way to expose a large group of employees to health and wellness programs and services offered by their employer. As these can be time consuming to setup and manage, our nurses are available to coordinate and implement a health fair for your organization. We also offer Workshops/Seminars in areas such as depression and managing stress, designed to empower employees to deal with these costly and potentially devastating conditions. We can also provide customized, company branded quarterly newsletters and associated materials discussing the latest health alerts, health care trends and proactive strategies.
