If you are not able to attend an onsite screening, you can still participate in this year’s wellness event by submitting information directly from your doctor’s office in addition to completing the Health Risk Assessment.

Instructions for Primary Care Physician (PCP) form participation:

1. Print the ISAAC PCP Form 2023 for completion by your PCP.

2. Submit the completed form via the email or fax provided on the form.

3. Complete your online Health Risk Assessment 2023. WPV will receive your results, however, we suggest you print a copy for your records.

Forms and assessments must be received by WPV before October 27, 2023 to be eligible for wellness incentives.
